Comments:
A monitoring inspection was initiated on 4/5/2024 and concluded on 4/5/2024. There were 17 children present with nine staff supervising and one program director and a kitchen staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed. The inspection started at 10:10 am and concluded at 12:45 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and date to be corrected? for each violation cited on the violation notice and return it to me within five (5) business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring gain; and 3) person or person(s) responsible for implementation.

Standard #: 22.1-289.035-B-4
Description: Based on record review the center failed to ensure that each staff authorize the child day center to obtain a copy of the results of a criminal history record information check registry from any state in which the individual has resided in the preceding five years.
Evidence:
Staff # 1 listed on the Sworn Statement of residing in another state in the past five years. The staff has a hire date of 1/29/24 and there are no out of state back ground checks from the state listed on the Sworn Statement.

Plan of Correction: 1. Obtain PA background check as soon as possible.
2. Obtain any out of state background checks prior to hire in the future.

Standard #: 8VAC20-780-140-A
Description: Based on record reviews the center failed to ensure that each child shall have a physical examination by or under the direction of a physician; before the child's attendance or within 30 days after the first day of attendance.
Evidence:
Child # 1, 2, 4 do not have a physical on file. All three have been in care more than 30 days.

Plan of Correction: 1. Message or in person talk to parents and send VA entrance physical paperwork to the ones who need it. Have them fax to the Dr. and back to me once filled out.
2. Make sure all new enrolls have this form filled out prior to enrollment or before 30 days.

Description: Based on record reviews the center failed to ensure that the child's records contain all the required information.
Evidence:
1. Child records # 2 & 4 do not have phone numbers for the physicians listed.
2. Child records # 1, 3, 4 do not have addresses for the emergency contacts listed.

Plan of Correction: 1. Have parents correct and put number and addresses in VA registration form.
2. Make sure parents of new enrollees have this filled in in the future.

Description: Based on record reviews the center failed to ensure that all children's records address Allergies and intolerance to medication or any other substances, and actions to take in an emergency situation.
Children's records 2, 4, 5 do not address if a child has an allergy or intolerance medication.

Plan of Correction: 1. Have parents correct and put N/A in the VA registration form, even if there are no allergies.
2. Make sure parents of new enrollees have this filled in in the future.

Standard #: 8VAC20-780-340-F
Description: Based on observation the center failed to ensure that all Children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence:
The reading corner has a bookcase that is approximately three feet high. When children are in the reading corner they are not in actual sight of the staff.

Plan of Correction: 1. Bookshelf will be turned against the wall instead of off the wall.
2. Rooms will be kept open to sight, and staff will ensure they have sight/sound visual of children at all times.
3. All teachers/staff.

Standard #: 8VAC20-780-500-A
Description: Based on observation the center failed to ensure that children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.
Evidence:
While in the two year old room the children finished eating lunch and started playing with the toys. The children did not wash their hands with soap and running water or wipes after eating lunch.

Plan of Correction: 1. Wipes will be available during lunch time, so when they are finished teachers can help wipe hands when others are waiting their turn to wash hands with soap and water.
2. Wipes will be available during/after lunch.

Standard #: 8VAC20-780-560-M
Description: Based on observation the center failed to ensure that No child shall be allowed to drink or eat while walking around.
Evidence:
Two children in the 2 1/2-3 year old classroom walked around drinking out of their sippy cups following lunch.

Plan of Correction: 1. Children will be made to sit while drinking out of their cups.
2. They will not be allowed to have cups unless they are seated.
3. All teachers/staff
