Comments:
An announced inspection was conducted on 4/16/24 from 9:30am to 11:30am. At the time of the inspection there were 22 children in care and 5 staff members. Children were observed participating in tabletop puzzles, coloring and other small group activities. Records were reviewed for 5 children and 5 staff members. Evacuation drills, emergency supplies and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Violations:
Standard #: 8VAC20-780-40-M
Description: Based on observation and interview, it was determined the facility did not ensure that the current list of children?s allergies, sensitivities, and dietary restrictions are maintained in a way that is accessible to all staff who work with children and kept confidential in each room or area where children are present.
Evidence:
1.The allergy lists posted in the classrooms and on the Parent Board showed children?s names, allergies and sensitivities in a manner that all people coming into the classroom or reading the Parent Board could see this confidential information

Plan of Correction: A cover sheet was immediately placed over the allergy list in each classroom. The Director will ensure allergy lists are kept confidential.

Standard #: 8VAC20-780-60-A
Description: Based on a review of children?s records and interviews, it was determined the center did not ensure that each child?s record contains all required information.
Evidence:
1.The record for child #2 did not include documentation of the father?s home address, phone number, workplace, and work phone number. The record for child #4 did not include the fathers work phone number.
2.The Director confirmed this information was not in the record.

Plan of Correction: The child's record was updated upon pick up. The Director will perform monthly audits to ensure all forms are completed properly.

Standard #: 8VAC20-780-70
Description: Based on a review of five staff records, it was determined that the center did not ensure a record was kept for each staff member with all the required information.
1.The record for staff #4 did not contain the address of the person listed as an emergency contact.
2.The Director confirmed the information was not in the record.

Plan of Correction: The teacher completed the emergency contact form. The Director will ensure staff have completed all forms during their first week of employment.

Standard #: 8VAC20-780-570-I
Description: Based on observation and interviews, it was determined the center did not ensure that a one-day's emergency supply of disposable bottles and nipples, appropriate for the children in care, shall be maintained at the center.
Evidence:
1.The infant room did not have a one-days emergency supply of bottles and nipples.
2.The Director confirmed the center did not have an emergency supply of bottles.

Plan of Correction: An emergency supply of bottles were ordered and placed in the infant room. A monthly audit will be conducted to ensure bottles are present.
