Technical Assistance:
The following was discussed with the owner, administrator and program director:

DIAPERING: None of the classrooms with non-potty trained children had running water next to the changing table and/or no changing table. It is recommended the center have a written procedure on how to reduce the risk of cross contamination before, during and after diapering when having to access water running outside of the classroom. Where there is no changing table in the classroom and children are to be taken to a bathroom to be changed, the center should have a procedure to ensure all supervision requirements are met at all times. and ensure all children are being supervised.

OUTSIDE PLAY AREA: It is recommended the gaps in the fencing are closed in and the overgrown weeds around the perimeter are trimmed. Before children go outside it is important for staff to remove any debris and dangerous items from the play areas.

Comments:
A initial inspection was conducted on 03/22/2024. A tour of the facility was conducted, in addition to a record review and interviews with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center, inside and outside were maintained in a safe condition.

Evidence:
1. The following hazards were on the outside play area
a. Razor blade and a hornets nest were observed on the ground.
b. Rust on a metal covering over the window of the center.
c. Small holes with jagged edges in the plastic fence
d. Covered storage area with no door had multiple nails protruding from the ceiling and
wall, a rusted metal strainer hanging on the wall and wires hanging from the ceiling and
wall.
e. The door leading into the building had areas of peeling paint.

2. The plumbing of the urinal in the one of the bathrooms had a rusted metal plate.

3. In the 16 month-2.5 year-old room the step riser had a piece of separate wood and was rough to the touch. There was also floor board separating from the wall.

Plan of Correction: All maintenance issues have been completed and the playground has been cleaned.

Standard #: 8VAC20-780-280-B
Description: Based on observation the center did not ensure all hazardous substances were kept in a locked place, using a safe locking method that prevents access by children.
Evidence:
In one of the bathrooms there was an unlocked cabinet containing Clorox wipes and a disinfectant spray bottle.

Plan of Correction: All chemicals were removed during the inspection and relocated to a locked area.

Standard #: 8VAC20-780-500-B
Description: Based on observation and interview the center did not ensure for children younger than three years, there was a nonabsorbent changing table or countertop designated for changing diapers.
Evidence:
There was not a changing table in the bathroom the director stated would be used to change the toddler's diapers.

Plan of Correction: A changing table has been ordered and will be placed in the bathroom for diapering.

Standard #: 8VAC20-780-500-C
Description: Based on observation the center did note ensure there was a t least a platform or step for toilet training children.
Evidence:
The bathroom that will be used to toilet train and change diapers did not have a platform or step to access the adult toilet.

Plan of Correction: A child size step stool has been ordered and will be kept in the bathroom for children to use.
