Comments:
An unannounced monitoring inspection was completed on 11/12/2019 from 10:00 A.M. until 3:00 P.M. At the time of the inspection 32 children were in care with seven staff and one administrator present. Six children's records, five staff records, required posted information, evacuation drills, six injury reports, injury prevention plan, first aid kits and supplies, parent handbook and outside play area were reviewed. Children were observed during diaper change, handwashing, free choice activities with blocks, dramatic play, cars, storytime, outdoor play, transition to lunch, lunchtime, and naptime. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-185-60-A
Description: Based on review of children files, the center failed to ensure that all required information was in a child's file.

Evidence:
1. Child #1's file did not contain a complete emergency address for one emergency contact. Date of enrollment was 09/15/2019.
2. Child #3's file did not contain a complete emergency address for one emergency contact. Date of enrollment was 09/03/2019.
3. Child #4's file did not contain a complete emergency address for both emergency contacts. Date of enrollment was 08/19/2019.
4. Staff verified that the records did not contain a complete address for emergency contacts.

Plan of Correction: Administration will take over reviewing children records to ensure all required information is obtained. Administration will contact the parents of the children missing the required information and update the files.

Standard #: 22VAC40-185-340-F
Description: Based on observation and interview, the center failed to ensure that children under 10 years of age shall always be within actual sight supervision of staff.

Evidence:
In Classroom #1 the Licensing Inspector observed a four year old child out of sight supervision of staff during nap time. Child #6 was laying on mat that was right inside the door at the entrance of the classroom. There was a shelf directly behind Child #6. Staff #3 was sitting on a carpeted area on the other side of the shelve patting the backs of two other children, and could not see Child #6.

Plan of Correction: Administration will talk with staff about rearranging the cots in the classroom so that all children are in sight supervision at all times.
