Discussed proof of birth documentation and start dates for children. Discussed playground maintenance regarding the mulch. Discussed the new water testing requirements.

An unannounced renewal inspection was conducted on 4/25/2024 from 9:30am to 12:00pm. At the time of entrance, 31 children were in care with 8 staff members present. Children were observed playing in centers, coloring, completing stem activities, lining up and preparing to go outside, playing outside, eating snack and washing hands. (4) staff records, (1)one agent?s record and (3) children?s records, the physical space, evacuation drills, medication, the injury prevention plan and attendance records were reviewed. The site was clean and organized. Interactions between the children and staff were positive. Walls and bulletin boards were decorated with artwork. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard #: 22.1-289.036-A Description: Based on interview and review of past background checks, the center's agent of an application did not have updated fingerprint results every five years.
Evidence:
1. Agent #1 had fingerprint results dated 5/17/2018 and has been more than 5 years.Plan of Correction: Agent #1 will have fingerprints redone at the earliest possible date, after using the wrong facility code to update fingerprints on April 12, 2024.
Standard #: 8VAC20-780-60-A-8 Description: Based on review of children's records, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Child B had a diagnosed food allergy and the center did not have a written care plan with instructions from a physician. The food allergy was listed as an allergy on the 1/10/2023 physical for the child.Plan of Correction: The school nurse spoke to mom of Child B and we now have an updated physical on file, signed by a physician and dated 1/10/2024, that states Child B has no allergies.
