Description: Based on a review of records and interview on September 2, 2021, the center did not ensure to obtain a copy of the results of a sex offender registry check prior to the first day of employment from any state in which the applicant has resided in the preceding five years.
Evidence: 1. The record of Staff #1 (DOH 7/6/21) did not contain documentation of results of an out-of-state sex offender registry check.
2. Administration acknowledged the sex offender registry was not completed.

Plan of Correction: When perspective staff identify that they have lived outside of the state of Virginia within the last 5 years we will be sure to review the Interstate Child Care Background checklist to ensure that all checks are submitted and received prior to hiring. an email submitted at 4:18pm after speaking with representative at DSSH Criminal Record Check Unit/Child Care Team in North Carolina requesting out-of-state documentation was forwarded to inspector.
