Standard #: 22VAC40-185-70-A Description: Based on review of 5 staff records, 1 record is not complete. Evidence: There is not documentation of reference checks for staff #1. Plan of Correction: I'm sure I did it, when I locate them I will put them in the file.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were accessible to children. Evidence: In the Pre K room, hazardous substance, (board cleaner) was located on a shelf, in an unlocked cabinet, accessible to children. Plan of Correction: Corrected during inspection, and going forward we will make sure to lock the cabinet after each use.
Standard #: 22VAC40-185-560-G Description: Based on observation, when food was brought from home, it was not clearly labeled. Evidence: In the PS3 classroom, a lunch box brought from home, was not labeled with child's name. Plan of Correction: Corrected during inspection and going forward I will remind staff to check for labels when food is brought from home.
