A announced monitoring inspection was completed with the director and one staff. Seven children were in are with an age range of seven to eleven years old. Children were observed during free play and snack. No children were on medication. All required paper work was posted. Two staff records were reviewed. Evidence: Child A with a start date of 8/11/18 was missing the place of employment and business phone number for both parents. Plan of Correction: We will ask the parents for the missing information
