An unannounced inspection was completed on April 29, 2019 from approximately 12:10p.m. to 3:15p.m. in response to a self-reported incident received on April 17, 2019 regarding forbidden actions. The inspection was initiated on April 18, 2019, collateral interviews were conducted off site on April 18, 2019, April 29, 2019 and May 7, 2019 and the inspection was completed on May 7, 2019. Staff, children and classrooms were observed, staff and collaterals were interviewed and policies, procedures and children's and staff records were reviewed related to the self-reported incident. The Director and Owner were available for the inspection and were present for the exit interview at which time the inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. There were no violations to support the self-reported incident.
