Technical Assistance:
Discussed not starting staff until receive results from fingerprinting. Each medication needs a medication authorization form. Center currently using medication authorization forms.

Comments:
An unannounced monitoring inspection was conducted from 9:00 am-11:00. There were a total of 67 children with 18 staff members present. Children were observed during breakfast, letters and letter work book. A selection of staff and children records, medications and other documents were reviewed. Violations:
Standard #: 22VAC40-185-500-B
Description: Based on observation, Disposable diapers are not being disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: The staff in the infant's class is disposing diapers in a trash can that is not foot operated.

Plan of Correction: We will buy a foot operated trash can.

Standard #: 22VAC40-185-510-C
Description: Based on a review of children medications, the medication form does not include duration of the parent's authorization for medication. Evidence: Child #1 has 2 long term medications at the center. There is not a medication form completed to include the duration of the parents authorization for one of the medications.

Plan of Correction: I will have the parent complete a medication form for the other medication.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of staff records, the center has not obtained results from fingerprinting for each staff before date of hire. Evidence: Two out of five staff records reviewed started working at the center prior to receiving the results of their fingerprints. Staff #1, date of hire 1/15/19 did not get results from fingerprinting until 1/23/19. Staff #2, date of hire 8/8/18 did not get results from fingerprinting until 9/7/18.

Plan of Correction: We will not start the staff until the fingerprint results are back.
