Comments:
The licensing inspector conducted an unannounced monitoring inspection on 05/02/2019 from 8:18am to 2:15pm. The children were observed playing in centers, playing on the playground, and eating morning snack. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Ten children?s records and ten employee records were reviewed during this inspection. Ten injury reports were reviewed. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 04/24/2019 Last shelter-in-place drill: 09/17/2018 Last fire inspection: 01/02/2019 Last health inspection: 01/14/2019 Today, the following child to staff ratios were observed: Infants ? 10:3 First Steps (Twelve to Eighteen months) ? 4:1 Toddler I ? 9:2 Toddler II (Twos) ? 9:2 Get Set (Twos) ? 16:2 Preschool I ? 13:2 Preschool II - 10:2 Pre-Kindergarten - 20:2 Kindergarten (Fours and Fives) - 14:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-160-A
Description: Based on review, the center did not ensure that two of ten staff records contained documentation of negative tuberculosis screenings 12 months prior to or 21 days after employment. Evidence: 1. The record of staff #1 (start date: 1/8/19) did not contain documentation of a negative tuberculosis screening. 2. The record of staff #5 (start date: 10/22/18) did not contain documentation of a negative tuberculosis screening within 12 months prior to or 21 days after employment. Staff #5 had a tuberculosis screening dated 11/27/2018.

Plan of Correction: Documentation for staff one has been obtained. TB screenings will be required within two weeks of hiring.

Standard #: 22VAC40-185-240-C
Description: Based on review, the center did not ensure two of ten staff records contained documentation of the required16 hours of annual trainings. Evidence: The record of staff #8 (start date: 1/21/15) had documentation of 7.25 hours of annual trainings. The record of staff #9 (start date: 8/22/16) had documentation of 2.75 hours of annual trainings. Program directors and staff who work directly with children should annually attend 16 hours of staff development activities related to child safety and development and the function of the center.

Plan of Correction: Training hours will be monitored closely. All staff will be required to obtain 16 hours annually.

Standard #: 22VAC40-185-560-F
Description: Based on observation and interview, the center did not ensure to follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: The inspector observed the center's staff serve rice cakes to the children in care. The center's snack menu listed rice cakes and juice for morning snack. The center did not serve juice or a substitute as a second meal component.

Plan of Correction: Our policy is to serve two components for snack. In the future we will be sure to serve the required snack components.

Standard #: 22VAC40-185-570-A
Description: Based on observation, the center did not ensure that protective belts were fastened securely when children are placed in a feeding table. Evidence: The inspector observed three infants sitting in a feeding table. All three infants were not securely strapped into their seat.

Plan of Correction: Infant faculty has been made aware of requirement. Directors will monitor.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review, the center did not ensure that two of ten staff records contained documentation of out of state child abuse and neglect search request by the end of the 30th day of employment. Evidence: 1. The record of staff #5 (start date: 10/22/2018) did not have documentation of an out of state child abuse and neglect search request. 2. The record of staff #9 (start date: 8/22/2016) did not have documentation of an out of state child abuse and neglect search request.

Plan of Correction: Out of state search requests have been sent. All out of state hires will have search requests done upon hiring.
