Comments:
The licensing inspector conducted an unannounced monitoring inspection Monday, May 13, 2019 from 03:30pm to 05:40pm. The children were observed arriving from school, doing homework, eating snack, and working on various academic enrichment activities. All classrooms, buses and the playground were inspected today. Medication is not administered. Seven children's records and seven employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 04/17/2019 Shelter-in-place drills: 04/17/2019 Last fire inspection: 04/22/2019 Last health inspection: 10/30/2018 Today, the following child to staff ratios were observed: School Age 1 (2nd & 3rd grade)-16:2 School Age 2 (4th grade)-16:2 School Age 3 (5th grade)-15:1 School Age 4 (6th grade)-16:1 If you have any questions about this inspection, please contact your licensing inspector, Roshon Casey, at (804) 662-9760.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 05/13/2019, the center did not ensure that one of the seven records reviewed contained the address of two designated people to call in an emergency if a parent cannot be reached. EVIDENCE: The record of Child #2 was missing the address for the second emergency contact listed.

Plan of Correction: Will contact parent and get address for emergency contact by this week.

Standard #: 22VAC40-185-540-C
Description: Based on observation on 05/13/2019, the center did not ensure that the required first aid kits contained the minimum items. EVIDENCE: The fist aid kit on the first bus inspected did not contain a thermometer, gauze tape, a cold compress, tweezers and scissors.

Plan of Correction: Corrected on site.

Standard #: 22VAC40-185-550-H
Description: Based on a review of documentation on 05/13/2019, the center did not ensure that the emergency document for vehicles contained information that pertain to each site frequently visited or of routes frequently driven by center staff for center business (such as field trips, pickup/drop off of children to or from schools, etc.) EVIDENCE: The emergency documents on the second bus inspected did not contain information regarding the routes traveled to pick up children from schools and drop off at home.

Plan of Correction: Will have information to driver by the end of the week.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of staff records on 05/13/2019, the center did not ensure to obtain a copy of the results of a search of the central registry maintained by any other state in which the individual has resided in the preceding five years within 30 days of employment. EVIDENCE: The record of Staff #3, whose date of hire was 12/01/2017, did not contain an out of state central registry finding

Plan of Correction: Will send out by the end of the week.
