An unannounced monitoring inspection was conducted between the 8:00am and 10:30am. Upon arrival there were 49 children in care with 10 staff present. A sample of 2 staff and 4 children's records were reviewed. The program director confirmed there were not children care with diagnosed food allergies at this time. Children were observed eating breakfast, playing with toys and transitioning from one classroom to another.

Standard #: 22VAC40-185-270-A Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition. Evidence: On the playground, parts of the chain on the swings were rusted. The swing hangers holding the right swing were opened. A finger could fit through the space. The Director was present and confirmed the rust and open swing hangers. Plan of Correction: The following corrective action was provided by the director during the exit interview: The director will have her maintenance person remove the rust and replace the swing hangers. In the meantime, children will not use the swings.
Standard #: 22VAC40-191-60-C-2 Description: Based on a record review the center did not ensure all staff had a central registry check within 30 days of employment. Evidence: Staff #1, hired on 10/22/19 and Staff #2, hired on 02/11/19, did not have documentation of having the results of a central registry finding. Plan of Correction: The following corrective action was provided by the director during the exit interview: Both check came back for missing information. The director documented re-sending Staff #1's check on 12/07/18 and Staff #2's on 03/18/19. The director will call today to follow-up with the results.
