Comments:
A joint investigation was conducted with the local department of social services to investigate a self reported incident regarding a forbidden action. An onsite inspection was conducted on April 12, four staff records were reviewed on April 18 and an exit meeting was conducted on May 14, 2019. Based on evidence obtained through classroom observations, interviews with staff, review of documentation and review of a video tape recording, the self reported incident could not be substantiated. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of four staff records, it was determined that not all staff had a tuberculosis (TB) test/screening within 21 days of hire or 12 months prior to employment. Evidence: For staff #4 (date of hire: 3/5/19) there was no documentation of a negative tuberculosis screening on file.

Plan of Correction: TB screening was completed on 5/7/2019 and is in her file.

Standard #: 22VAC40-185-70-A
Description: Based on a review of four staff records, it was determined that the facility did not have all required information in each staff record. Evidence: For staff #4 (date of hire: 3/5/19) there was no documentation of education, experience, or two references as to character and reputation as well as competency before employment.

Plan of Correction: The two references will be finished.

Standard #: 22VAC40-191-60-B
Description: Based on review of four staff records, it was determined that not all staff had a completed sworn statement or affirmation before employment. Evidence: Staff #4 (date of hire 3/5/2019) did not have documentation of a sworn statement in their file.

Plan of Correction: The sworn statement was completed on 3/1/2019. It was in her HR file. A copy has been made.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of four staff records, it was determined that not all staff had Central Registry Search results within thirty days of employment. Evidence: Staff #4 (Date of hire: 3/5/2019, did not have documentation of a Central Registry search report on file.

Plan of Correction: The Central Registry Search was located in the HR file. The results will be in her file as soon as possible.
