An uannounced monitoring inspection was conducted beginning at 3:50 p.m. and ending at 5:00 p.m. There were 21 children in care ranging in age from 6 years to 11 years old with 3 staff. Children were observed lined up to wash hands before snack. Snack today was banana, whole wheat crackers and water. During snack, staff read a story to the children. After snack, children washed hands and prepared to go to the gym for physical activity due to the rain this afternoon. Four children's records were reviewed. One emergency medication was reviewed. Emergency drills were reviewed and up to date. Two staff records were reviewed prior to the inspection. Standard #: 22VAC40-185-510-A Description: Based on review of one emergency medication, the center failed to ensure they obtained written permission from the parent or guardian. Evidence: Child #1 has an epinephrine injector on site and the long term authorization was completed but did not include a signature from the parent or guardian to administer. Plan of Correction: This form will be updated tomorrow morning.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of staff records, the center failed to request an out of state central registry background check within 30 days of employment. Evidence: Staff #2, date of hire 7/26/2018, indicated out of state residence in North Carolina on a Sworn Statement dated 7/24/2018 but there was no documentation of an out of state central registry background check request for North Carolina in the record. Plan of Correction: The out of state central registry background check will be requested today.
