Today we discussed emergency equipment

Comments:
An unannounced monitoring inspection was conducted today. There were 18 children in care with three staff including the site director, assistant staff and one junior counselor. Children were observed interacting in table top activities. Children's records were reviewed. Staff emergency contact information was reviewed. Staff records were reviewed at the central location during an earlier inspection. Required postings, annual inspections, medication, first aid kit and emergency items were reviewed. Violations regarding emergency contact information, annual inspections and emergency items were identified and appear on the violation notices. Each violation was reviewed with the site director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and interview the center failed to ensure that each staff record contains all required information. Evidence: 1 - During record review there was not an emergency contact form available for review for staff 2. a. Staff 2 stated that he has been working at this center location since December 14, 2018.

Plan of Correction: Corrected during the inspection. Going forward center management will ensure that each staff member has an emergency form filled out and on file on the first day of employment.

Standard #: 22VAC40-185-540-E
Description: Based on observation and interview the center failed to ensure that emergency items are working. Evidence: 1 - The battery-operated radio was not working during the inspection. 2 - Staff 1 confirmed that the radio was not working. Additionally, the flashlight could not be located.

Plan of Correction: Going forward center management will ensure that required emergency equipment is in working order including extra batteries for the radio and flashlight. Furthermore, both the radio and flashlight will be stored in a consistent location so that it is easily accessible whenever it is needed.

Standard #: 22VAC40-80-120-E-1
Description: Based on observation and interview the center failed to ensure that the violation notice is posted in the center. Evidence: 1 - The most inspection violations are not posted in the center. 2 - Staff 1 confirmed that the most recent violation notices are not posted in the center.

Plan of Correction: Center management will ensure that the most recent violations are posted in the center each day, during all hours of operation.
