Comments:
An unannounced monitoring inspection was conducted on 5/22/19 from 11:10am until 2:10pm. At the time of entrance, there were 74 children in care with 15 staff present. A sample of 8 children's records and 7 staff records were reviewed. Children were observed eating lunch, playing outside and resting quietly during nap time. Infant feeding and diapering, handwashing and table sanitizing procedures were also observed. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Three violations in the areas of administration and special care provisions and emergencies are identified on the violation notice and were discussed with the center director during the exit interview.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review and interview, the center did not ensure that documentation that each child has received the immunizations required by the State Board of Health was obtained before the child can attend the center. Evidence: 1. Child 1 has a date of enrollment of 10/29/18. Documentation of immunizations for child 1 were not obtained by the center until 11/20/18, which is after the child's first day of attendance. 2. The center director confirmed that documentation of immunizations for child 1 was not obtained until after the child's first day of attendance.

Plan of Correction: The center responded with the following: Going forward, the center will ensure that documentation of current immunizations are obtained before the child's first day of attendance.

Standard #: 22VAC40-185-140-A
Description: Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. Child 2 has a date of enrollment of 9/10/18. Documentation of a completed physical examination was not obtained by the center until 11/2/18, which is more than 30 days after enrollment. 2. Child 3 has a date of enrollment of 2/1/18. Documentation of a completed physical examination was not obtained by the center until 3/21/18, which is more than 30 days after enrollment. 3. The director confirmed that documentation of a completed physical examination for child 2 and child 3 were not obtained within the required time frame.

Plan of Correction: The center responded with the following: Going forward, the center will ensure documentation of physical examinations are obtained from the parent within the required time frame.

Standard #: 22VAC40-185-510-G
Description: Based on a review of medication being stored at the center, the center did not ensure that written authorization from a parent or guardian is obtained in order to administer medication to a child that would normally be administered by a parent or guardian. Evidence: 1. There are three emergency medications being stored at the center for child 4 with a written doctor authorization but no written parental authorization. 2. The director confirmed that there is no written parental authorization for the center to administer the prescribed medication for child 4.

Plan of Correction: The center responded with the following: The director will ensure the parent signs the parental authorization forms today at pick-up.
