Comments:
An unannounced monitoring inspection was conducted today. There were 18 children in care ages six to 11 years old with five staff. Children were observed during outside play, transitions, and restroom routines. Children and staff emergency contact information was reviewed. There were two violations identified during the inspection and appear on the violation notice. These violations were discussed with the site director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-510-C
Description: Based on record review and interview the center failed to ensure that medication authorizations are current. Evidence: 1 - The medication authorization for child 1 expired on December 20, 2017. However, the medication was present in the center medication lock box. 2 - Staff 1 was unaware that the medication on site had expired. 3 - Staff 1 confirmed that the medication authorization on site has expired.

Plan of Correction: Center management will either obtain a new authorization or the medication will be returned to the parent.

Standard #: 22VAC40-185-560-F
Description: Based on observation and interview the center failed to ensure that USDA regulations are followed when snacks are provided by the center. Evidence: 1 - Children were served "cheezit" crackers and water for snack. Therefore, the children were served one component were two components are required. a. The posted menu lists today's snack as "cheezits" and water. 2 - Staff 1 confirmed that the children were served the crackers and water for snack today.

Plan of Correction: Center management will ensure that when snack is provided by the center two components will be served and listed on the posted menu.
