Today's unannounced focused monitoring inspection was completed on 05/30/2019 from 9:50 A.M.-11:30 A.M. The purpose of the inspection was for determining compliance to previous violations. There were 24 children in attendance with five staff, one administrative assistant, and one administrator present. The records for seven staff were reviewed, six over the counter medications were reviewed, and an observation in the classrooms took place. The theme for the week is Science and Nature. The children were observed during morning work cycle which included art activities, building, and making and testing paper airplanes. Exceptions to compliance are noted on the violation notice. Standard #: 22VAC40-185-160-C Description: Based on review of staff records, the center failed to obtain the results of a follow-up tuberculosis screen every two years after the first initial screening or testing. Evidence: 1. Staff #4's last documented tuberculosis screening was dated 03/09/2017. 2. Staff #5's last documented tuberculosis screening was dated 03/06/2017. 3. Staff #6's last documented tuberculosis screening was dated 03/06/2017. 4. Staff verified the dates of the tuberculosis screenings and that they were the most current on file. Plan of Correction: Appointment will be made this week for staff to obtain a tuberculosis screening or test.
