Technical Assistance:
We discussed the requirements for infant formula. The risk assessments for violations were provided. We reviewed the center procedures for maintaining resilient surfacing.

Comments:
Thank you for your assistance during today's unannounced renewal inspection conducted from 11:05 AM to 4:20 PM. Today, there were sixty-one children, ages infancy through age eight years, with fourteen to sixteen staff. I viewed program activities, daily routines, equipment, posted information, agency inspection reports, insurance, staffing, supervision, interactions with children, emergency supplies and drill records, transportation/field trips, injury records, diapering procedures, diaper ointment, sunscreen, infant care and records, medication, six records for children, business entity background checks, seven staff records, staff qualifications, staff training, indoor and outdoor areas. The children had access to age appropriate toys, equipment and materials. The teachers have implemented a variety of planned activities for learning and group discussions. They have been learning about pets and the circus. The school age children have have weekly activities for music, art, Spanish, and cooking. A two year license will be recommended. Standard #: 22VAC40-185-570-C
Description: Based on a review of the records of three infants fed formula at the center, and interviews with staff, the center failed to have written information on the brand of formula and feeding schedules for two infants. Evidence: Staff indicated that two of the infants were solely on breast milk before parents started bringing bottles with a mix of commercial formula and breast milk, and now formula for child 1F, and a switch to only formula for child 2F. The feeding record in the classroom that was completed by parents only had information about breast milk and staff indicated it was not updated. The staff wrote sticky notes which were posted to follow and based on what parents verbally told staff for the feeding schedules.

Plan of Correction: The teacher will have the parents updated the feeding information for the records of child 1F and 2F. The director will make sure all morning staff understand the procedures for having parents update the feeding schedules and adding the brand of formula whenever there are changes.

Standard #: 22VAC40-185-570-I
Description: Based on a review of formula for three infants, and interviews with staff, the center did not maintain a one-day's emergency supply of commercial formula for one infant. Evidence: There were only pre-made bottles of formula for the day for child 1F. When checking the storage bin for child 1F, a staff member indicated that there had been a canister of formula kept at the center and it was not located.

Plan of Correction: The teacher will request and obtain an emergency supply of formula for child 1F and help parents understand that the extra supply for the center is required.
