Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the home.

A subsidy health and safety inspection (SHSI) was initiated on October 27, 2021 and concluded on October 28, 2021. There was one child present with one caregiver supervising. A monitoring inspection was also conducted on this date.

Violations:
Standard #: 22VAC40-665-160-B
Description: Based on interview and review, provider did not ensure there is a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: The attendance log did not show any children present on 10/26/2021 and 10/27/2021 although provider stated children were present on 10/26/2021 and one child was present during inspection on 10/27/2021.

Plan of Correction: Attendance was noted during inspection.

Description: Based on review, provider did not ensure 1 of 2 children's records has documentation of required information.

Evidence: The record for child #2 did not have documentation of parent's employment phone numbers.

Plan of Correction: Provider requested information from the parents and information was added to the record.

Standard #: 22VAC40-665-190-B
Description: Based on review and interview, provider did not ensure caregiver has updated TB screening/test every 2 years.

Evidence: The record for caregiver had a most recent TB screening dated 6/25/2018 which is more than 2 years old. Caregiver stated she has had a screening/test since then but could not locate the documentation.

Plan of Correction: Provider will have screening or test completed.

Standard #: 22VAC40-665-410-B
Description: Based on interview, provider did not ensure a record is maintained of the dates of the practices drills for one year.

Evidence: Provider stated she has completed shelter-in-place and lockdown drills monthly but she has not documented the dates.

Plan of Correction: Provider will start documenting the dates and note what type of drill was done each month.

Standard #: 22VAC40-665-440-D-1
Description: Based on interview, provider did not ensure Virginia state statutes about safety belts and child restraints are followed as required by the Code of Virginia.

Evidence: Provider stated that she has transported child #1 (age 6) and child #2 (age 4) on multiple occasions in a vehicle without using the required child restraint devices appropriate for each child. This occurred as recent for both children on 10/26/2021 and child #1 was transported on 10/27/2021.

Plan of Correction: Provider secured car seat/booster seat from parents and will use whenever children are transported.

Standard #: 63.2-1720.1-A
Description: Based on review and interview, provider did not ensure required background checks are updated every 5 years.

Evidence: The record for provider had documentation of a central registry check completed 8/9/2016 which is more than 5 years old. Provider stated she has not had another central registry check completed since that date.

Plan of Correction: Provider stated she will fill out the search and mail it.
