Standard #: 22VAC40-665-540-A Description: Based on review, the vendor did not ensure that one of five staff records contained documentation that the staff was evaluated by a health professional and issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Evidence: The record of staff #4 (start date: 04/06/18) did not have documentation that the staff was evaluated by a health professional and issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Plan of Correction: The doctor said he will fax the TB results which were completed in 2018 as soon as the electricity is turned on [due] to a neighborhood electric power outage.
