Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 7:05 AM and departed at 8:20 AM. Two children were in care with two staff. Morning arrivals and activities were observed. It was reported that there are currently no children with medication. No children's injuries were reported. A sample size of five children's records were reviewed. An exit meeting was conducted with staff 1 prior to closure of the inspection.

Standard #: 22VAC40-185-130-A
Description: Based upon review of five children's records and staff interview, the facility has not ensured that they have obtained documentation of the required immunizations before the child attends the facility. Evidence: 1. The record for child 1 did not include documentation of immunizations. 2. Staff 2 verified that child 1 has been attending the facility since the beginning of the school year. 3. Staff 2 verified that documentation of immunizations was not in the record for child 1

Plan of Correction: The facility responded with the following: Master children's records are kept at the main office. Documentation of immunizations will be placed in all children's files.

Standard #: 22VAC40-185-140-A
Description: Based upon review of five children's records and staff interview, the facility has not ensured that each child's record includes documentation of a physical examination before the child's attendance or within month month after attending. Evidence: 1. The record for child 1 did not include documentation of a physical examination. 2. Staff 2 verified that child 1 has been attending the facility since the beginning of the school year. 3. Staff 2 verified that documentation of was not in the record for child 1

Plan of Correction: The facility responded with the following: Master files are kept at the main office. We will ensure that all required information is in the files at the facility site.

Standard #: 22VAC40-185-60-A
Description: Based upon review of five children's records and staff interview, the facility has not ensured that each child's record includes all required information. Evidence: 1. The record provided for child 1 did not include the child's first date of attendance . The record included only one of the required two designated emergency contact persons. 2. The record provided for child 3 included only one of the required two designated emergency contact persons. 3. The record provided for child 5 did not include the street address for one of the two designated emergency contact persons. 4. Staff 2 verified that the above information was not contained in the files.

Plan of Correction: The facility responded with the following: Master records are maintained at the main office. We will ensure that all children's records are complete with all requjired information at all times.
