An announced monitoring inspection was conducted on June 6, 2019 to review staff records. The licensing inspector reviewed administration, staff qualifications and training, background checks and licensure and registration procedures. The sample size consisted of four staff records. Areas of noncompliance are identified on the Violation Notice.

Standard #: 22VAC40-185-160-A Description: Based on record review, in one of four staff records reviewed, the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis (TB) screening no later than 21 days after employment and it shall have been completed within 12 months prior to employment. Evidence: Program Coordinator verified that there was no documentation of a TB screening in the record for Staff #2 (hire date 2/16/19). Plan of Correction: The staff member has completed the screening and will have it read tomorrow.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review, in one of four staff records reviewed, the licensee did not ensure that staff submit to fingerprinting and obtain results prior to employment. Evidence: Program Coordinator verified that there was no documentation of fingerprint results for Staff #4 (hire date 5/16/19). Plan of Correction: Staff member has scheduled the appointment.
