Comments:
An unannounced Monitoring Inspection was conducted today from TIME. There were 69 infants to 5 year olds directly supervised by 13 staff. The physical plant, 4 staff records, 10 children?s records, 3 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in circle time, playing freely with other children or the teachers, and playing outside on the playground. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Standard #: 22VAC40-185-60-A
Description: Based on review, the facility failed to ensure that the center maintains and keeps a separate record for each child enrolled with the required information. Evidence: Reviewed 10 children's records and found that Child 1, 6, 7, 9 and 10 were missing documentation of parent work number and/or place of employment.

Plan of Correction: We will correct it by the end of this week.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to ensure that areas and equipment of the center, inside and outside shall be maintained in a clean, safe and operable condition. Evidence: 1) Observed that here was a broken wooden window blind plank accessible to children in the far window of the 3s classroom. 2) Observed in the Pre-K classroom, 2 large loops from the window cords, above the children's shelves, accessible to children.

Plan of Correction: The window blind plank will be fixed by the end of June and the window blind cords were corrected at the time of the visit.

Description: Based on observation, the facility failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: Observed in the Preschool 2 classroom, three bottles labeled "soap & water", "sanitizer" and "cleaner disinfectant" stored in a classroom cabinet that was not locked with a locking method that prevents access by children.

Plan of Correction: Corrected at the time of the visit.

Description: Based on observation, the facility failed to ensure that cleaning and sanitizing materials shall not be located above food, food equipment, utensils or single-service articles and shall be stored in areas physically separated from food. Evidence: Observed in Preschool 1, Borax, stored above food, food equipment, utensils and single service articles.

Plan of Correction: Corrected at the time of the visit.

Standard #: 22VAC40-185-540-C
Description: Based on observation and staff interviews, the facility failed to ensure that they had the minimum required items in the first aid kit. Evidence: Observed that the first aid kit only had 1 triangular bandage.

Plan of Correction: We will purchase additional bandages by the end of the day.
