An unannounced monitoring inspection was completed today 6/6/2019 The inspection began at 7:45 am and concluded at 12:30 pm. There were 35 children present with six staff members during the inspection. During the inspection the entire physical plant was viewed, arrival, infant care was observed, lunch time was observed, emergency supplies were viewed, nap time was observed, and policies and procedures were reviewed, five children's records and three staff's records were reviewed. Discussion with the director/owner: repainting cribs where the paint is peeling, and staffing changing the mailing address for the business, adding a changing table to the toddler room as required by the standard 500b, and attendance.

Standard #: 22VAC40-185-140-A Description: Based on review of five children's records, the center failed to ensure that all physicals examinations were obtain within 30 days of enrollment. Evidence: 1. Child # 1 was enrolled in the program on 11/5/2018 and on the day of the inspection did not have a physical examination. Plan of Correction: The director will obtain a physical exam today. In the future all physical will be obtained prior to starting.
Standard #: 22VAC40-185-150-B Description: Based on review of five children's records, the center failed to ensure that all immunizations were signed and dated by a physician or designee. Evidence: 1. Child # 1 did not have a signed immunization record from a physician or designee. Plan of Correction: The director will require new immunizations with a signature and in future will review prior to accepting.
Standard #: 22VAC40-185-340-F Description: Based on observation, the center failed to ensure that all children under the age of 10 year of age were in actual sight and sound supervision of staff. Evidence: 1. Staff # 3 walked out of her classroom across the hallway to the infant room to obtain child # 5 and left children # 1 and # 4 in the toddler classroom by themselves. Children # 1 and 4 were out of sight and sound supervision for approximately one minuet. Plan of Correction: The director will remind staff to stay within sight and sound supervision of children at all times and will monitor.
Standard #: 22VAC40-185-430-A Description: Based on observation of the toddler room, the center failed to ensure that all equipment and materials were appropriate size for children using it. Evidence: 1. Observed in the toddler room (17 months to a new 2 year old), new equipment and materials that were not age and stage appropriate for the age of the children using it. There were several pieces of toys that were small enough to be considered a choke hazards in the toy basket and on the floor. Plan of Correction: The director and staff will go through all toys to ensure they are safe.
