Technical assistance was provided regarding injury report documentation.

An unannounced monitoring inspection was conducted today from 10:15am-11:45am. There were 8 children directly supervised by 2 staff between ages 2 and 12. The physical plant, programming, menu, 2 staff records, 4 children's records, evacuation drills, injury reports, emergency supplies, and policies were inspected. The director stated no medications are on site today. All staff present are CPR/First Aid certified. Children were observed playing with trucks, playing on the slide, making an octopus craft and playing video games. Drop off was observed and parents were able to choose between several different lunch options. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-140-A Description: Based on record review, 1 of 4 children's records reviewed did not contain documentation of a physical examination prior to the child's first day of attendance or within 30 days of starting at the center. Evidence: 1. Child #2 (Date of enrollment: 1/14/19) did not have a physical examination on file. Plan of Correction: We are getting the physical today and putting it in the child's file.
Standard #: 22VAC40-185-240-D-5 Description: Based on record review and staff interview, there was not at least one staff member on duty who has obtained instruction in performing the daily health observation of children within the last 3 years. Evidence: 1. Staff #1 and Staff #2 last completed daily health observation training on 5/18/2016. Plan of Correction: We found a training online and will have this completed by the end of next week.
