The licensing inspector conducted an unannounced monitoring inspection Wednesday, June 5, 2019 from 4:30pm to 5:30pm. The children were observed playing relay games such as simon says and red light, green light, and playing with balls in the gym. Five children?s records and four partial employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 05/08/2019 Last shelter-in-place drill: 05/08/2019 Today, the following child to staff ratios were observed: School Age ? 16:4 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-40-E Description: Based on observation and staff interview on 06/05/2019, the center did not ensure that the center's services were maintained in compliance with the terms of the current license issued by the department. EVIDENCE: 1. Child #6 was observed in attendance at the center during the inspection. 2. Staff #3 stated that Child#6 is three years old. 3. The center's license issued by the Virginia Department of Social Services is for children age five to 12 years old. Plan of Correction: Will not have child to come to site anymore.
