Standard #: 22VAC40-665-540-A Description: Based on review of five staff files, the facility failed to have documentation of tuberculosis test or screening that the individual is free of communicable tuberculosis at the time of employment and prior to coming to coming into contact with children. Evidence: Staff 1 (date of hire 3/29/2019) and Staff 4 (date of hire 5/15/2019) did not have documentation of a tuberculosis test or screening prior to coming into contact with children.
