Comments:
A subsidy health and safety inspection was completed on June 6, 2019 from approximately 10:15a.m. to 12:30p.m. to review supplemental health and safety requirements. Thirty-six children were in care and five staff were on premises during the inspection. Staff and children were observed in classrooms engaged in activities to include center time, free play, playing a game, transitioning to lunch and lunch time. Interviews and interactions were conducted and medications were reviewed. Five children?s records and five staff records were reviewed. The Owner/Director was available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: 22VAC40-665-650-E
Description: Based on observation, the licensee failed to develop a written policy and procedure that describes how each group of children receives care by consistent staff or team of staff members.
Evidence: 1. There were no written consistent care procedures to review. 2. Administrator indicates that while the center does practice and implement consistent care, procedures have not specifically been written.

Plan of Correction: The Director will add written consistent care procedures to the center's polices and procedures.

Standard #: 22VAC40-665-780-A-3
Description: Based on observation, the licensee failed to practice at least annually lockdown procedures.
Evidence: 1. A review of the practice drill logs revealed no documentation of a lockdown drill for 2018. 2. Administrator #1 acknowledged that a lockdown drill was not completed in 2018.

Plan of Correction: The Director will ensure that a lockdown drill is completed annually.
