Comments:
A Subsidy Health and Safety inspection was conducted on 6/5/2019 to review supplemental health and safety requirements. The inspector arrived at 7:00 AM and departed at 8:20 AM. Morning arrivals and activities were observed. A sample size of five children's records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice

Violations:
Standard #: 22VAC40-665-520-B
Description: Based upon review of children's records and staff interview, the facility has not ensured that each child's record includes information on allergies, including food allergies, and actions to be taken in an emergency situation. Evidence: 1. Staff 2 showed the inspector an allergy chart indicating that child 1has a food allergy to cashews, pecans and tree nuts. 2. The parent indicated on the enrollment record of child 1 that he is allergic to cashews, pecans and tree nuts. 3. Child 1's physical examination by a physician did not indicate any food allergies. 4. The child's record did not indicate any actions to be taken in an emergency situation. 5. Staff 2 stated that there was no medication for child 1and that they were waiting for the parent to provide an allergy management plan for child 1.

Plan of Correction: The facility responded with the following: Information will be obtained regarding the food allergies for child 1 and, if diagnosed by a physician, an allergy management plan will be obtained and any needed medication.

Description: Based upon review of records and staff interview, the facility has not ensured that they have obtained a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. The parent indicated on the enrollment forms for child 1 that child 1 is allergic to cashews, pecans and tree nuts. 2. Staff 1 verified that child 1 has a food allergy. 3. There is no written allergy care plan, to include instructions from a physician regarding the allergy and steps to be taken in the event of an allergic reaction for child 1.

Plan of Correction: The facility responded with the following: Staff will speak with the parent to obtain all required documentation and to ensure that staff know what action must be taken in the event of an allergic reaction.
