Comments:
An unannounced monitoring inspection was conducted with center director on June 11, 2019 to determine compliance with the ,inspection. There were 28 children present, ranging in age from 19 months to 9 years old. The children were having rest time and waking up for afternoon snack. Two staff files and five children's files were reviewed - please see Supplemental Summary for details. Reminder: conduct a shelter in place drill soon - two are required every 12 months. There are no medications on site today; one medication goes home with the child each day and the child is absent today. One staff is certified in MAT and will take MAT-Diabetes soon. Standard #: 22VAC40-185-80-A
Description: Based on review of the attendance sheets, the facility failed to mark every child in attendance on the written attendance record each day. Evidence: only one attendance record with 9 children marked present could be located at 12:30 p.m.; 28 children were present.

Plan of Correction: Center staff will complete an attendance record daily.

Standard #: 22VAC40-185-240-D-1
Description: Based on review of staff files and staff interview, the facility failed to have all of the required training to administer medication. Evidence: Child 6 takes prescribed diabetic medications, but there are no staff certified in the "Medication Administration Training - Diabetes".

Plan of Correction: Staff 1 will take the Medication Administration Training- Diabetes as soon as a class can be scheduled.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of two staff records, the facility failed to have each staff hired prior to January 23, 2018 to submit to fingerprinting no later than October 1, 2018. Evidence: Staff 2 (date of hire 3/14/2016) did not have documentation of a fingerprint check.

Plan of Correction: Staff 2 will get a fingerprint check in the next 10 days.
