Comments:
An unannounced monitoring inspection was conducted today from 10:20am-2:10pm. There were 174 children (infants-5yrs.) directly supervised by 31 staff. The physical plant, 10 staff records, 10 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, being bottle fed, napping,playing with legos and blocks, playing with play-doh, playing outside, and eating lunch. Today's lunch consisted of whole grain cheese melt, tomato alphabet soup, fresh fruit and milk. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. Areas of non-compliance are identified on the Violation Notice. Description: Based on observation, children's information was not kept confidential. Evidence: In the Hummingbirds classroom an allergy list that contains children's names and their food allergies was posted, the information was not covered or hidden to the public.

Description: Based on record review, the center did not obtain documentation that two or more references were verified before employment. Evidence: The records of Staff#1, Staff#3, Staff#7, and Staff#8 were missing two references as to their character and reputation, as well as competency required prior to the first day of employment.

Evidence: In the Bluebirds classroom, a bottle of disinfectant was observed inside an unlocked upper cabinet. In the Chickadees classroom, a bottle of bleach an water was observed unlocked on top of the paper towel dispenser.
