Standard #: 22VAC40-185-510-E
Description: Based on a review of medication for four children, and interviews with staff, the center did not ensure that prescription medication for one child was labeled as required with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Evidence: A plastic container with emergency medication for child 4 only had the child's name written on it and was not in the original container with the prescription label.

Plan of Correction: The parent was immediately contacted and arrangements will be made to obtain a container with the prescription label for child 4. The executive director will review medication requirements with M.A.T. certified staff who accept medication from parents to make sure they understand the requirements for medication.

Standard #: 22VAC40-185-570-C
Description: Based on a review of formula feeding records for infants, and interviews with staff, there was required information missing for three children's records. Evidence: Child 1F had a feeding schedule record for feeding whole milk and no written information for commercial formula. However, there was a container of emergency formula stored at the center and the infant teacher indicated that they feed child 1F infant formula from bottles. Chid 2F had a feeding schedule record for breast milk. However, the infant teacher indicated that they feed a mix of breast milk and commercial formula to child 2F and the brand of formula and feeding times/amounts were not added to the record. The feeding schedule record for child 3F only stated "formula" and "breast milk" and did not include the brand of commercial formula, times and amounts. The staff indicated that feeding schedules are posted on a wipe board in the room and not maintained as part of individual records.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of nine records for staff, and an interview with administrative staff, the center failed to obtain a Virginia central registry finding for two staff within thirty days of employment. Evidence: Staff member 8 began work on March 12, 2019 and there were only criminal history record checks obtained for the record. Staff member 9 began work on February 26, 2019 and there were only criminal history record checks obtained for the record. A second type of criminal history check had been obtained instead of the central registry finding for each.

Plan of Correction: The executive director immediately provided the request forms for the Virginia central registry checks to staff member 8 and 9. The center administration will make sure the findings are obtained, viewed and maintained in the file. The executive director will make sure all types of required background checks are obtained within the required time frames when new staff are employed.
