Comments:
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, June 18, 2019 from approximately 8:30am to 10:00am. The Director was present and assisted the inspector throughout this inspection. There were a total of 21 children being supervised by two staff and one volunteer. During the inspection, children and staff were observed participating in a variety of activities. The children were observed eating their morning snack, taking bathroom breaks, and interacting with peer and staff in the gym. Staff were observed providing supervision and positive guidance throughout the inspection. All areas of the facility used for childcare including two classrooms, the gym, hallways, and bathrooms were inspected. The facility is equipped with toys and supplies and items are available to the children. Required postings were on display in the center. Per the Director, there are no medications on site at this time. During the inspection, five children's records and five staff's records were reviewed. The last documented health inspection was 07/16/2018. The last documented fire inspection was 07/05/2018. There are various staff members with current CPR & First Aid training along with Daily Health Observation training. Description: Based on a review of five staff records on 06/18/2019, the center did not ensure that one staff resubmit tuberculosis (TB) test results every two years. Evidence: 1) The TB test results in the record for Staff #1 (DOH: 02/04/2016) were dated 04/28/2017 and expired on 04/28/2019. Each staff member shall resubmit TB test results every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department.

Plan of Correction: Per the Center: Staff #1 will resubmit TB test results in the next two weeks.
