An unannounced Complaint Inspection was conducted on June 5, 2019 in response to a complaint that was received by the Virginia Department of Social Services, Licensing Division. Interviews were conducted with staff relating to allegations of Lack of Supervision, Care and Protection and Staff to Child Ratios. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegation of Care and Protection, and the complaint is determined to be VALID Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 business days. Standard #: 22VAC40-185-340-A Complaint related: Yes Description: Based on review, the facility failed to ensure that when staff are supervising children, they shall always ensure their care, protection, and guidance. Evidence: Reviewed the injury reports for the 2s classroom and found that there were 13 incidents of Child 1 being bitten by Child 2. 2 of the incidents were not noticed by staff but were noticed by parents when they found bite marks on Child 1's body later that day. All incidents were documented between January 18, 2019 through May 7th, 2019. Incident reports state that staff will "shadow" Child 2 to prevent further occurrences, but Child 1 still was continuously bitten, by the same child. On 3/21/19 and 3/22/19, Child 1 was bitten 3 times within 2.5 hours. Plan of Correction: Staff are working on shadowing the children involved, and had discussed a plan with the parents as well as added an additional staff member to the classroom.
