Comments:
The licensing inspector conducted an unannounced focused inspection on Tuesday, June 11, 2019, to confirm that the provider is in compliance with resilient surfacing standards. The inspector was on site on June 11, 2019 from 01:30pm to 2:57pm. Interviews with staff were completed and observations were made at the center. The inspector observed and measured areas of the playground that were in use. There was one violation resulting from this inspection. Violations:
Standard #: 22VAC40-185-330-B
Description: Based on observation and interviews, it was determined that there was an insufficient amount of resilient surfacing in the use zone of the playground equipment and the area surrounding equipment did not encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Evidence: 1. There was approximately 4 to 9 inches of rubber mulch underneath the preschool/school age swings. The swings required 6 inches of rubber mulch. The inspector interviewed a staff member who revealed that the children use the preschool/school age swings. The inspector observed a sufficient amount of mulch collected in piles on the playground but the mulch had not been spread to ensure that all areas under and around the swings had sufficient resilient surfacing. 2. The inspector observed children playing on the slide underneath the wooden deck. In the past the center reported that the slide was not in use. There were two wooden posts on each side of the slide. The post on the left side of the slide measured about 3 feet from the slide and the post on the right of the slide measured about 2 feet 8 inches from the slide. There should be six feet on all sides of the slide.

Plan of Correction: 1. I will have the rubber mulch spread out before the children are allowed to play on the swing set. 2. Effective immediately children are not allowed to play on the slide. I have already notified home office that it needs to be removed from the playground.
