Technical Assistance:
We discussed the requirements for diapering procedures. The risk assessments for violations were provided. Also, we reviewed the requirements for maintaining the mulch in fall zones of playground equipment, staff positioning during nap time to ensure sight supervision, and lead teacher training to meet qualification requirements.

Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 10:52 AM to 5:40 PM. Today, there were one-hundred and thirty-five children, ages infancy through ten years, with twenty-three staff. I viewed program activities, staffing, supervision, interactions with children, equipment, posted information, agency reports, daily routines, diapering procedures, infant care and records, diaper ointment, sunscreen, medication, ten records for children, thirteen staff records, staff qualifications, staff training, injury records, emergency supplies and drill records, menu, nutrition, indoor and outdoor areas. The children were provided a variety of age appropriate activities and outside experiences. The teachers have provided time for individual choices for activities of interest to children in the care and group led activities such as music, art and stories. The staff were observed providing frequent interactions with children and responding to individual needs for comfort and guidance. Standard #: 22VAC40-185-500-A
Description: Based on observation of the diapering procedures at the center, and an interview with staff, the staff have not always washed their hands with soap and running water before immediately after a diaper change. Evidence: In the older infant/younter toddler room, a staff member was seen changing a diaper and then obtaining cabinet keys from another staff member in the room. The keys were carried to the sink counter and put down while the staff member then washed hands followed by unlocking the cabinet.

Plan of Correction: The director reminded the staff to always obtain all needed items before beginning a diaper change and to wash hands immediately after the diaper change and before handling items used in the classroom by multiple staff. The director will review diapering procedures with all staff. The director or assigned staff will schedule times for regular observations of diapering procedures to make sure staff follow all required diapering procedures and prevent the possible spread of illness.

Description: Based on observation of the diapering procedures by staff, and an interview with staff, all diaper disposal procedures were not followed by use of a foot operated pedal to open the container lid or in such a way that neither the staff member's hand nor the soiled/wet diaper touched an exterior surface of the storage system during disposal. Evidence: In the older infant/younger toddler room, a staff member was observed hand lifting the lid to the container to dispose of a used diaper, wipes, gloves and paper towels used after a diaper change. It was confirmed by staff that the foot pedal was operable.

Plan of Correction: The director spoke with the staff who change diapers in the infant/toddler area to make sure they will always use the foot pedal for disposal of items during diaper changes. The administrative staff will review the diapering procedures with all staff. The director or assigned staff will schedule times for regular observations of diapering procedures to make sure staff follow all required diapering procedures and prevent the possible spread of illness.
