An unannounced renewal inspection was conducted on 7/1/19 from 10:35am until 1:15pm. At the time of entrance, there were 14 children in care with 5 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed participating in morning activities, eating lunch and resting quietly during nap time. Hand washing and restroom procedures were also observed. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and center postings were reviewed. The center does not currently provide transportation and there have been no injuries to children in care since the previous inspection. Three violations in the areas of administration, programs and special services are identified on the violation notice and were discussed with staff 2 during the exit information.

Standard #: 22VAC40-185-70-A Description: Based on record review and interview, the center did not ensure that staff records contain all the required information. Evidence: 1. Staff 1 was hired at the center as a lead teacher. The record for staff 1 does not contain written information to demonstrate that she possesses the education required by the job position. 2. Staff 1 confirmed that she did not provide documentation of her educational qualifications when hired at the center. Plan of Correction: The center will obtain documentation of staff 1's educational qualifications.
Standard #: 22VAC40-185-440-J Description: Based on observation and interview, the center did not ensure that there shall be at least thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib. Evidence: 1. The crib occupied by child 1 in the infant room had approximately 12 inches of space between it's service side and the service side of an adjacent crib, where 30 inches are required. 2. Center staff confirmed that there were not 30 inches of space between the service side of child 1's crib and an adjacent crib. Plan of Correction: This was corrected during the inspection. Going forward, staff will ensure there are 30 inches of space between service sides of occupied cribs and other furniture.
Standard #: 22VAC40-185-570-E Description: Based on observation, the center did not ensure that prepared infant formula shall be dated. Evidence: 1. There were 4 bottles in the infant room that were labeled with the child's name but were not dated. 2. Center staff confirmed that the infant bottles were not dated. Plan of Correction: The bottles were dated with today's date during the inspection. Going forward, staff will ensure infant bottles are dated.
