Comments:
The licensing inspectors conducted an unannounced inspection on May 29, 2019 between 11:30 am to 1:35 pm in response to a self-reported incident the department received on May 23, 2019 regarding supervision. During the inspection, interviews were conducted with staff, observations were made in the center, and documentation was reviewed. There were violations cited related to the incident. Violations:
Standard #: 22VAC40-185-340-A
Description: Based on interview with staff, review of documentation, and observation, the center did not always ensure care, protection, and guidance when supervising children. Evidence: 1. Interviews with Staff#1, Staff#2, Staff#3, and Staff#4 confirmed incident on May 2, 2019 involving Child#1, age 5 being unsupervised by staff after transition between TCW classroom 1 and TCW classroom 4, approximately 70 feet apart. Child#1 exited the facility alone to the playground area, which is approximately 80 feet from the exit door. 2. Incident report and facility documentation dated May 2, 2019 confirmed incident involving Child#1 being unattended by staff after procedures were not followed during transition between TCW classroom 1 and TCW classroom 4. 3. Written staff statements provided by Staff#1, Staff#2, Staff#3, and Staff#4 detailed their accounts and involvement pertaining to the incident on May 2, 2019. This information was confirmed during staff interviews conducted on May 29, 2019.

Plan of Correction: All classroom staff will receive active supervision training during the pre-service period for the upcoming school year. August 2019 Revised procedures for child transition will be provided to classroom staff as well as training on how to implement those procedures. Unannounced monitoring visits will be conducted twice a month with a specific focus on transition and safety procedures implementation. Ongoing Sept 2019-May 2020

Description: Based on interview with staff and review of documentation, the center did not ensure children under 10 years of age are always within actual sight and supervision by staff. Evidence: In an incident on May 2, 2019, Child#1, (age 5) was found without staff supervision outside near the playground area. 1. Staff#2 stated Child#1 was first observed outside of the building coming to the playground area alone. 2. Staff#1 stated Child#1 was asked "Did a teacher bring you out?" and Child#1 replied, "no." 3. Staff#1 stated Staff#3 and Staff#4 were questioned about Child#1 being unattended outside. Staff#3 revealed Child#1 asked to go back to her classroom and "I walked up to the door and opened the door to let her go without supervising". Staff#4 stated Child#1 was let out of my classroom by Staff#3. 5. Facility documentation dated May 2, 2019 confirmed Staff#3 allowed Child#1 to walk out of the classroom and did not keep her in sight. 6. Written statements by Staff#1, Staff#2, Staff#3, and Staff#4 confirmed the incident on May 2, 2019. 7. Incident report dated May 2, 2019 outlined the sequence of events and communication history between staff.

New transition procedures will include a required sign in/out sheet for individual child transitions.
