Comments:
An unannounced monitoring inspection was conducted from 1:00 - 4:00 pm with the youth director. There were 98 children, ranging from six years to 15 years of age, present with 19 staff supervising. Children were observed playing group games in the gym, working on an arts and crafts projects, participating in a group meeting and returning from the swimming pool. Ten children records and seven staff records were reviewed and in compliance. The emergency escape drill log was observed and in compliance. Eleven medications and authorization forms were reviewed. Standard #: 22VAC40-185-510-D
Description: Based on review of 11 medications, the center did not have written authorization to administer medication to a child in care. Evidence: The center did not have a long-term medication authorization form signed by a physician and parent for Child #2, child #4 and child #7's medication.

Plan of Correction: Contacted parents and requested the missing forms during the inspection.
