Comments:
An unannounced inspection was conducted July 15, 2019 from approximately 1:45 pm until 5:00 pm. Upon entrance there was 1 staff member with 6 children. There were additional staff on site for administrative purposes. The sample size of records reviewed included 4 children's records and 4 staff records. There are no medications being administered as a part of this program. The climbing wall is not being used as apart of the outdoor play area. Areas of noncompliance are noted on the violation notice. The results of the inspection were reviewed and verified with Administrative Assistant. Please contact the licensing inspector with any question (757) 404-0568.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of 4 staff records and staff interviews, the licensee did not ensure each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: The record for Staff #3 did not include documentation of obtaining a TB screening. Staff #3 began working as a staff member in the program on 6-3-19.

Plan of Correction: Staff #3 will get the TB screening and place in the record.

Standard #: 22VAC40-185-70-A
Description: Based on review of 4 staff records and staff interviews, the licensee did not ensure in each staff record there was written information to demonstrate the individual possesses the education, orientation training, staff development, certification, and experience required by the job position. Evidence: In the Pre-K room there were 6 children and 1 staff present, staff #1. The documented job title for Staff #1 is lead teacher (program leader) for the Pre-K room and there is not documentation in the record for education/ training that indicates Staff #1 is qualified as the program leader.

Plan of Correction: The staff will complete training this week or find documented training and place it in the record.

Standard #: 22VAC40-185-340-D
Description: Based on record review, observation and staff interviews, the licensee did not ensure that in each grouping of children there was at least one staff member who meets the qualifications of a program leader or program director. Evidence: In the Pre-K room there were 6 children and 1 staff present and there is not documentation of at least one person who meet the qualifications for program leader.

Plan of Correction: The staff will complete training this week or find documented training and place it in the record. Another staff who is qualified will work in the classroom until training is obtained.

Standard #: 22VAC40-185-430-C
Description: Based on observation and staff interviews, the licensee did not ensure that play equipment used by children shall meet all the requirements. Evidence: Five preschool children were observed playing on the outdoor swings. The S-hooks on the swings were reviewed and it was determined that 8 out of 16 of the S-hooks were open more than the thickness of a penny.

Plan of Correction: The maintenance staff will fix it and the swings will be off limits until fixed.
