An unannounced monitoring inspection was completed on 7/17/19. There were 11 children present with three staff. Five children's records and three staff records were reviewed. No medications were being administered at the center. The inspector arrived at the center at 1:15 pm and departed at 3:30 pm. The children were observed during nap time and during outside time. There was discussion with the summer coordinator and the director about the following: medications, first aid kit items for off site trips, daily health observation update training, pick up authorization for children.

Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that all required information was documented in each child's record. Evidence: 1. The record for child #1 was missing the address for one of two emergency contacts. 2. The record for child #3 was missing the following required information: the work phone number for the mother and the father of the child, the address for two of two required emergency contacts, proof of identity for the child. 3. The record for child #5 was missing the address and phone number for one of two emergency contacts for the child. Plan of Correction: The site coordinator will contact the parents and document all missing information.
Standard #: 22VAC40-185-260-B Description: Based on review of inspection reports and interview, the center failed to ensure that there was an annual approval from the health department. Evidence: 1. The last approval from the health inspector was completed 3/29/18. Plan of Correction: The director will have the health inspector complete the "Report of Environmental Sanitation Inspection" form.
