Comments:
The licensing inspector conducted an unannounced renewal inspection on 7/16/2019 from 2:35pm to 5:20pm. The children were observed eating snack and playing in centers. A formal observation was completed in two preschool classrooms. Snack consisted of pretzels and juice. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms were inspected today.

Five children?s records and five employee records were reviewed during this inspection. Medication was reviewed by the inspector. Two injury reports were reviewed as well as the center?s injury prevention plan, which was updated 12/2018.

The center's first aid kit and emergency supplies were inspected.

Today, the following child to staff ratios were observed:
Infant and Toddlers - 5:2
Twos - 3:1
Twos and Threes - 7:2
Fours and Fives - 9:1
Fours and Fives - 11:2
School Age - 6:1

The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. The inspector observed three windows with peeling and chipped paint on the outside of the building in the playground area within the reach of children. 2. There was a torn cushion located on the playground that exposed foam, which presented a choking hazard.

Plan of Correction: 1. We contacted the contractor and they said it will be completed. 2. Corrected during the inspection. The cushion was removed from the playground.
