Comments:
The inspector conducted an unannounced renewal inspection on 07/26/2019 from 09:26am to 2:25pm. The children were observed playing in centers, playing in the gymnasium and eating lunch. Lunch consisted of chicken nuggets, corn bread, bananas, peas, carrots and milk. The menu was posted. A formal observation was completed with the five year old and infant classrooms. Interviews were held with staff throughout the inspection, and the inspector interacted with children when appropriate. All areas of the center were inspected as well as the center's mini bus. Nine children's records and six staff records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 07/17/2019 Last fire inspection: 10/19/2018 Last health inspection: 02/28/2019 Today, the following child to staff ratios were observed: Fives - 5:1 Fours and Fives - 3:1 Trans. Twos - 7:2 Twos - 7:2 Threes and Fours - 11:2 Mobile Infants - 2:2 Infants - 7:2 Toddler - 8:2 School Age (Group 5) - 6:1 School Age (Group 2) - 3:1 School Age (Group 3) - 7:1 School Age (Group 1) - 7:2 School Age (Group 4) - 4:1 The violations from the previous inspections were checked for corrections. There was one repeat violation found during today's inspection. The Department has not yet received a complete renewal application from the center. A complete renewal application must be received by the Department prior to expiration of the license on August 31, 2019 or the license cannot be renewed. Standard #: 22VAC40-185-210-A
Description: Based on record review, the center did not ensure that one of six staff records contained documentation of proof of education for qualified program leaders. Evidence: The record of staff #1 (start date: 3/3/19) did not have documentation of the staff's proof of education.

Plan of Correction: We have requested staff #1's proof of education during the inspection. We will get it and place in the staff's file.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. The Group 1 classroom had an unlocked closet with several chemicals inside. The classroom did not have any children in care at the time. 2. Classroom # 6 contained an unlocked cleaner out of the reach of children in care. 3. Classroom #7 contained an unlocked cleaner within the reach of children in care.

Plan of Correction: Corrected during the inspection.
