Comments:
An unannounced renewal inspection was conducted on July 17, 2019 from approximately 11:00 AM to 2:15 PM to determine the center's compliance with licensing standards. The director was present for the inspection. The staff to child ratios were deemed in compliance with licensing standards. The census today consisted of 76 children. The following ratios were observed in the classrooms: Infants 2:4, Older Infants 2:8, 2 years 2:13, 3 years 2:11, 4 years 2:16, 5-6 years 2:13, 7-8 years 1:11. Lunch today consisted of taco salad with ground beef, lettuce, tomato and cheese; tortilla chips; cucumber slices; watermelon; and milk. The staff were observed to be engaged with the children in age appropriate activities and to be meeting the needs of the children in care. All areas of the facility including the classrooms, outdoor play areas, bathrooms, and the bus were inspected. The facility had the following posted: license, last inspection, emergency numbers, evacuation maps/procedures, daily schedule, and various parent information. The first aid kit and non-medical emergency supplies were inspected and found complete. The last evacuation drill was conducted on 6/27/19. The last shelter-in-place drill was conducted on 3/19/19. The Emergency Preparedness plan is current. Health Inspection: 5/21/19. Fire Inspection: 2/15/19. Medications and authorizations were reviewed. There were multiple staff present who are certified in medication administration, daily health observation, First Aid, and CPR. Seven children's records were reviewed. The licensing inspector reviewed seven staff records and agent records at the administrative office on July 18, 2019 from 9:45 AM to 11:30 AM. Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of seven staff records, two records did not contain an Out of State Child Abuse and Neglect Search Request. Evidence: 1. The updated Sworn Disclosure Statement indicated Staff #4 (hire date 4/15/19) and Staff #5 (hire date 5/28/19) lived out state within the past 5 years. 2. The records for Staff #4 and Staff #5 did not contain Out of State Child Abuse and Neglect search requests.

Plan of Correction: The center will ensure that out of state child abuse and neglect search requests are sent in a timely manner for all staff who have lived out of the state within the past 5 years.

Description: Based on review of the agent records, the center did not obtain a national fingerprint-based criminal history record check for one listed agent. Evidence: The record for Agent #1 contained documentation of a national fingerprint-based criminal history record check dated 11/27/18. A statement of eligibility based on a national fingerprint-based criminal history record check was required to be obtained prior to 9/30/2018.

Plan of Correction: The center will ensure background check requirements are met in a timely manner.
