An off site review of background checks for business agents/representatives was conducted between 11:15 AM and 1:00 PM. Upon receipt of the remaining required background checks, please submit the findings to the Valley Licensing Office. This information is needed for the recommendation of the renewed license. As a reminder, compliance with all applicable licensing standards and the Code of Virginia is the responsibility of the licensee. Thank you for your cooperation. Standard #: 63.2(17)-1721.1-B-2 Description: Based on an off site review of the background checks for seven business entity agents/representatives, and an interview with administrative staff, the center failed to obtain all required criminal history record information for two agents/representatives of the business entity. Evidence: The only criminal history records for BE1 and BE2 were the Virginia Criminal History Name Searches completed on October 24, 2017 with the state police. Both business entity representatives did not submit to fingerprinting for the results of background check findings with the Federal Bureau of Investigations and the Virginia State Police that were due by September 30, 2018. Plan of Correction: The executive director immediately contacted BE 1 and BE 2 about the need to make appointments with a fieldprint location for fingerprints to be sent to the Office of Background Investigations. The results will be obtained for the center records. When received, the results will be forwarded to the licensing inspector. In the future, administrative staff will make sure all required background checks are in compliance and obtained on time.
