Staff shall submit verification of required fingerprinting results within the next 10 days.

Comments:

An unannounced full renewal inspection was conducted today. There were 9 children in care, with a total of 3 staff members assigned to their direct supervision. Children's records, emergency preparedness items, background checks, staff training, staff records, the physical plant, children's nutrition, administrative documents, and classroom programming were inspected today. The children were observed playing outside, playing with small motor skills equipment, practicing animal recognition skills, and engaging in group classroom conversations. The children were to be served cauliflower chicken soup, vegetable salad, mashed potatoes, chicken meatballs, and milk for lunch later today. This inspection was completed between approximately 9:45am and 11:30am. Thank you for meeting with me today. Standard #:	22VAC40-185-60-A
Description:	Based on a review of children's records, it was determined that three records were not complete. Evidence: 1 - Child record C does not include: the mother's work phone number; the address for one emergency contact person; documentation showing that an acceptable form of proof of identification was viewed by a staff person. (documentation indicates that a form that is not on the acceptable list was viewed and accepted as proof of identification.) 2 - Child record D does not include: documentation showing that an acceptable form of proof of identification was viewed by a staff person. (documentation indicates that a form that is not on the acceptable list was viewed and accepted as proof of identification.) 3 - Child record F does not include the address for one emergency contact person.
Plan of Correction:	Each parent will be asked for the missing/correct information and the records will be updated.
Standard #:	22VAC40-185-520-C
Description:	Based on a review of ointment documentation, it was determined that the application of diaper ointment has not been recorded as required. Evidence: It was reported to the Inspector that staff do not record the application of diaper ointment when applied to children in care.
Plan of Correction:	We rarely apply diaper ointment. Moving forward, we will record it when we do apply it.
Standard #:	22VAC40-185-520-D
Description:	Based on a review of ointment documentation, it was determined that the application of insect repellent has not been recorded as required. Evidence: It was reported to the Inspector that staff do not record the application of insect repellent when applied to children in care.
Plan of Correction:	We only apply bug spray on the days that we go outside later in the afternoon. We will begin recording it when bug spray is applied.
Standard #:	22VAC40-185-550-D
Description:	Based on a review of emergency preparedness items, it was determined that the center has not conducted an emergency evacuation drill each month. Evidence: The center did not conduct a fire evacuation drill during the month of June, 2019.
Plan of Correction:	We will do a fire drill every month.
Standard #:	22VAC40-191-60-G-1
Description:	Based on a review of staff background checks, it was determined that one fingerprint background search was not completed as required. Evidence: The fingerprint search for staff A was not completed by the current employer. This person's search was requested and completed by a previous employer, with that employer's identifying information included on the search findings report.
Plan of Correction:	A new fingerprint search will be completed, using the codes for this school.
