Technical Assistance:
An exit interview was conducted with the lead staff to discuss the results of today's inspection.

Comments:
An unannounced monitoring inspection was conducted today, July 22, 2019 to determine the summer camp's compliance with licensing standards. The inspection was initiated at 3:00pm and concluded at approximately 5:35pm. Upon arrival, seventy-six school age children were observed being supervised by eight staff. Children were observed reading, playing board games, and various games in the gymnasium. All areas used by children were inspected to include the main hallways, children's restrooms, classrooms, gymnasium, outdoor play areas to include the soccer fields, volleyball field, playground equipment area, and open grass areas. The center had the required postings displayed on the portable bulletin board upon entering the classroom. The center's first aid kit and emergency supplies were inspected with the required supplies found. Seven children's records and five staff emergency records were inspected with the required information present. The following was also inspected on this day: annual inspections, fire/shelter-in-place drills, parent agreements, injury reports, and other pertinent information. Description: Based on the inspection of the children's playground the required amount of resilient surfacing was not present under the playground equipment. Evidence: During the inspection of the children's playground, it was discovered the large playground equipment did not not have the required amount of resilient surfacing under the circular monkey bars and blue dual slide. The mulch was found to be compacted in various areas with only approximately 3 inches of mulch.

Plan of Correction: Per lead staff: We will have additional mulch added to the playground equipment.
