Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 11:00 AM and departed at 1:45 PM. Forty-five children were in care in the gymnasium, with another 12 children arriving from a field trip during the inspection. Activities, swimming and lunch service were observed. A sample size of nine staff records and six children's records were reviewed. Injury records and medication administration were reviewed. An exit meeting was conducted with the program director and supervisor prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances such as cleaning materials are kept in locked locations using safe locking methods that prevent access by the children. Evidence: At 11:20 AM, the custodian was observed entering the men's bathroom to clean, leaving the custodial cart with aerosol cleaner, spray toilet bowl cleaner and a mop bucket of soapy water unattended in the gymnasium where the children were playing and eating lunch. The cart was unattended for five minutes while the custodian was in the men's bathroom. It was unattended again when the custodian took the mop into the bathroom to mop the floor.

Plan of Correction: The facility responded with the following: The program director will speak with the custodial crew again to remind them of requirements regarding cleaning materials.

Standard #: 22VAC40-185-510-A
Description: Based upon review of medication administered to children in care and staff interview, the facility has not ensured that medication is administered only with parental authorization. Evidence: 1. Records indicate that albuterol was administered to child 1 on July 15, 2019 and July 24, 2019. The signed authorization from the parent indicated that the authorization expired on July 1, 2019.

Plan of Correction: The facility responded with the following: All staff with Medication Administration Training (MAT) will be reminded to carefully check all authorizations and expiration dates prior to administering any medication. The albuterol will be returned to the family of child 1 as child 1 is no longer attending care.
